Flat spot on cold start

On cold start, after letting the engine idle for 10s or so, I gently drive off. 20 yards or so down the road it’s as if there’s fuel starvation and the car hiccups badly, but doesn’t stall. Thereafter no problems at all.

Any idea what this may be symptomatic of?
